Individual and Non-Member School Registration Form
The following form will allow you to register up to 25 individuals for the Fall 2019 conference. We understand that some larger schools may be registering more than 25 faculty and staff. If this is the case, complete the first 25 registrants on this form. Then submit a second form with any additional registrants.
We are going paperless this year! Please be sure to provide email addresses for all participant so conference communications may reach all attendees.
Please note that on the day of the conference, workshop sessions will be filled on a first-come, first-served basis. Please provide feedback on the number of faculty/staff interested in each workshop. This will help us to locate each workshop in a place with adequate space and seating. Thank you!
Conference Registration Return Policy:
Refunds are available upon request only. Fees will be refunded in full if canceling prior to 10/11/19. If canceling after 10/11/19 and up to 10/18/19, the school or registrant will be refunded 50% of registration fee. After 10/18/19 the school or registrant is fully responsible for payment and no refunds will be issued.
For schools using Title II Funds:
Title II funds are now disbursed after the conference has taken place. In order to qualify for early bird pricing, schools will need to pay in full for the conference, and then apply for their Title II Funds. Once MSC has received payment for the town, the school will be reimbursed for the Title II distribution. Thank you!
